• Inform us of any phone number, insurance, or address changes
• Come wearing a mask that covers both your mouth and nose
• If under 18, a guardian must be present
• If you've had Covid-19 symptoms in the past two weeks, please bring proof of a negative Covid test result
• If you are not feeling well, please reschedule your visit
